Employees’ Provident Fund Organization (EPFO) has issued EPF/ EPS withdrawal forms like 5A, 6A, 9, 10C, 10D, 12A, 13, 14, 19, 20, 31. Each member/ employee shall nominate a person in the declaration form i.e, EPF withdrawal form-2. Because, the nominated person has to receive the benefits/ amount in case of death of the working employee/ EPF Member.
These forms were being used by members/ employees in a certain conditions only in the cases of (i) Purchasing a house or dwelling a house (ii) for repayment of loans in special cases (iii) an other miscellaneous cases. EPF Form 2: Nomination and declaration form for unexempted/ exempted category candidates.
EPF Form 13: This form is useful for transferring the Provident Fund account of a member from one establishment to another establishment or when an employee switches companies/ employers.
EPF Form 14: This application form for financing a life insurance policy out of Provident Fund account.
EPF Form 10C: This form is for claiming to refund the employer’s share or the member who is less than 58 years of age receiving a scheme certificate retention of membership or for availing the settlement benefit through Employee Pension scheme or Old family pension fund. This form can be used along with Form 19/20.
EPF Form 10D: This form is useful to get the pension either by a member/ a nominee after death of PF member or widow/ widower.
EPF Form 19: This form is to be submitted by a member to withdrawal his Provident Fund dues on leaving service/ retirement/ termination.
EPF Form 20: After death of any member, this form is to be used by a nominee/ family member to claim the member’s Provident Fund.
EPF Form 31: This form helps the member to withdraw funds in the form of advance/ load/ for medical/ for purchasing house, etc.
EPF Form 5 (IF): This form is preferred by the person eligible to receive Employees’ Provident Fund dues of the deceased member who died while in services.
